Please answer the following questions to order your personalized calendar. Note: You may use as many photos and/or art work as you would like for each month (10 or less works best). So I know which months the photos are to appear in, please write the month on the back of each photo. Be sure to send your list of personal dates! Click here for date list form. Liability for lost or damaged photos limited to one roll of replacement film. If your photos are irreplaceable, please make duplicates and send those to me or you may send me a diskette or CD containing your photos in .jpg or .gif formats only. NO slides or negatives. All items will be returned with your order.

Lorri's Personalized Calendars reserves the right to return orders unprocessed due to acts of God or picture/photo content.
